If you would like to send in a sample for testing, please complete the Laboratory Services Agreement. Once this is received at the lab an account will be created for you.
Each sample sent in needs to be accompanied by the BEL Chain of Custody Form. Simply add your contact information, sample date/designation, and the tests you require. You can also add any project name/PO# that is necessary for your reference. The sample size needed will vary based on the testing being performed.
